We have a new medication form this year for your child’s physician to complete if they will be taking medication via the school nurse. Please keep in mind we cannot take any medication without this form.

Also any over the counter medication can only be kept in the nurse’s office for 2 weeks maximum. After the two weeks if the medication is not picked up by the parent or guardian it will be discarded. If you wish for your child to keep an over the counter medication in the nurse’s office for more than two weeks, a PPA form will have to be completed by the physician.

Any medication brought to the school nurse must be signed in by the parent or guardian and signed out by them as well. Medication that needs to be picked up by the parent or guardian cannot be sent home with the student.
